Charles Anthony Fornaszewski

Funeral services for Charles Anthony Fornaszewski, 79 of Sallisaw, Oklahoma will be at 2:00 PM on Tuesday, July 14, 2020 at Bethel Baptist Church in Sallisaw, Oklahoma. Burial will follow at Sallisaw City Cemetery under the direction of Agent Mallory Martin Funeral Service, Inc.

He was born on March 23, 1941 in Hartshorne, Oklahoma to Mildred (Miller) Fornaszewski and Anthony Charles “Tony” Fornaszewski and passed away on July 9, 2020 in Fort Smith, Arkansas. He was married to Betty Jean (Brooks) Fornaszewski on September 3, 1960 in Sallisaw, Oklahoma. She preceded him in death on January 1, 1996. Mr. Fornaszewski was a DHS Supervisor for 33 years and founder of the Child Welfare Advisory Board. He was an active member of the Jaycees, a Deacon at Bethel Baptist Church, a Black Diamond fan and one of the founders of Sallisaw Black Diamond Booster Club. He loved playing golf and spending time with his grandsons.

Survivors are:

2 daughters: Stacey Jean England and Gayland of Sallisaw, OK and Toni Renee Jasna and Chad of Sallisaw, OK;

1 brother: Stanley Fornaszewski of Sallisaw, OK;

3 grandsons: Treye Girdner and Natalie of Sallisaw, OK, Dylan Jasna of Sallisaw, OK and Dalton Jasna of Sallisaw, OK;

2 great grandchildren: Quaid and Cash Girdner of Sallisaw, OK;

2 nieces: Christina Hornsey of Fort Smith, AR and Patti Barnes of Fort Smith, AR.

He was preceded in death by his wife: Betty Jean and 1 brother: John Fornaszewski.

Active Pallbearers will be Chad Jasna, Gayland England, Treye Girdner, Dylan Jasna, Dalton Jasna and Chris Sharp.

Honorary Pallbearers will be John D. Ely, Dale Brown, Marvin Thouvenel, Randy Edwards and “The Round Table of Coffee”.

Viewing will be 2 – 8 pm Sunday and 10 am – 4 pm Monday at Agent Mallory Martin Funeral Home in Sallisaw and 6 – 8 pm Monday at Bethel Baptist Church. The family will receive friends 6 – 8 pm Monday at Bethel Baptist Church.
